Surgical Consultation (new and existing patients)

To secure your appointment, we kindly require that you provide a valid credit card for booking purposes. All new surgical consultations require a consultation fee at the time of booking that will be used towards your total at time of service. Consultation fee may vary surgeon to surgeon. Consultation fee will not be refunded unless the appointment is cancelled with more than 24 hours notice. Consultation fee is not required for follow up appointments.

• A 20% scheduling deposit is required to reserve a surgery date for any cosmetic procedure. The scheduling deposit is non-refundable. In the event that you cancel or reschedule your surgery, the 20% scheduling deposit will be forfeited.
• The balance of the scheduling deposit is due at the time of your pre-op appointment. We accept cash, check, care credit, and all major credit cards.
• If your surgery is cancelled less than 72 hours prior to the scheduled date and time of your surgery, 100% of the fees collected will be surrendered to PryorHealth as part of the cancellation fee. Cancelations in surgeries made 4 to 14 days prior to the scheduled surgery will results in a cancellation fee equal to 50% of the fees collected. Cancellations made 15 to 21 days prior to the scheduled surgery will result in a cancellation fee equal to 25% of the fees collected. All cancellations and rescheduling requests will result in the loss of your 20% scheduling deposit.

Medspa & Non-Surgical Procedures (new patients only)

To secure your appointment, we kindly require that you provide a valid credit card for booking purposes. All new medspa treatments require a $100 consultation fee at the time of booking that will be used towards your total at time of service. Consultation fee will not be refunded unless the appointment is cancelled with more than 24 hours notice. Consultation fee is not required for follow up appointments.

If you do not cancel your appointment within 24 hours, the consultation fee will not be refunded. If you need to cancel your appointment, please contact your designated Patient Care Coordinator or call (561) 776-9555.
